Hazel holds a master’s degree from California State University, East Bay (formerly California State University, Hayward), equipping her with advanced knowledge and a strong academic foundation in her field. She is a highly credentialed professional, holding two licenses as a Licensed Clinical Alcohol and Drug Counselor (KY&CA). In addition, she has earned certification through the International Certification & Reciprocity Consortium (IC&RC), reflecting her commitment to maintaining the highest standards of professional excellence in addiction counseling and behavioral health services.
As the Founder and Chief Executive Officer of Bridge the Gap Addiction and Mental Health Services, PLLC, Hazel has played a pivotal role in addressing the complex needs of individuals facing substance use and mental health challenges. Under her leadership, the organization has provided compassionate, client-centered care designed to empower individuals on their journey toward recovery and self-sufficiency. Her vision has always been rooted in closing service gaps and ensuring that underserved populations have access to quality care and supportive resources.
Hazel’s professional journey is marked by resilience, adaptability, and an unwavering dedication to community impact. She has successfully navigated the evolving landscape of behavioral health services, making strategic decisions to restructure and expand services in ways that best meet the needs of those she serves. Her transition toward developing sober living environments reflects her forward-thinking approach and her continued commitment to fostering safe, supportive spaces for long-term recovery.
In addition to her professional and entrepreneurial accomplishments, Hazel is deeply committed to her faith and ministry. She was licensed and ordained by Dr. Cynthia D. James of Landmark Ministries in Oakland, California. She currently serves as an Associate Minister under the leadership of Pastor Anthony Clark at Mount Hermon Christian Church in Louisville, Kentucky, where she continues to provide spiritual guidance, support, and encouragement to her community.
Hazel is also an active businesswoman engaged in various service-based ventures, including notary services and community support initiatives. Her entrepreneurial spirit is complemented by a strong work ethic and a deep understanding of the importance of trust, reliability, and professionalism in every endeavor she undertakes.
